Thank you for deciding to enroll with A+ Arts Academy for the 2026-27 school year! To start your application please fill out the form below.

Enroll Via Mail

Once the above form is completed you will receive email instructions on completing the rest of the application. One of our staff members will get in touch to assist you if you have any questions. If you would like the option to directly print a copy for yourself and then download the below enrollment form. Once you have completed the printed application, please send the completed application to tjohnson@aplusarts.com drop off a printed copy at 1395 Fair Avenue, Columbus, OH 43205 on Monday-Thursday 9-2pm. Documents required for enrollment are:

  • State-issued identification (parent and/or student)

  • Student's birth certificate

  • Proof of address (i.e. current utility bill, current lease, bank statement, etc.)

  • Proof of income (students 18 and over)

  • Current vaccination record

  • Student's official transcripts

  • Withdrawal letter from student's most recent school previously attended

  • Proof of guardianship, if applicable
